RCMS has been submitting COVID-19 immunization records in accordance with the California Department of Public Health (CDPH) submission guidelines. Those guidelines did not include a process to enter cell phone and email information. Those two elements are now required. To request a digital COVID-19 vaccine record please visit: https://myvaccinerecord.cdph.ca.gov/. If you requested a digital COVID-19 vaccine record and received a message that the information you provided does not match information in their system please complete this troubleshooting form https://myvaccinerecord.cdph.ca.gov/unmatched and submit to CDPH. There have been recent requests of RCMS Providers to provide diving physicals. RCMS DOES NOT have any physicians trained in dive medicine and all patients will be referred to Divers Alert Network (DAN). DAN is a group of not-for-profit organizations dedicated to improving diving safety for all divers. DAN maintains a comprehensive online library of free diver health, medical and safety resources. In addition, your DAN membership provides access to the industry's premier dive accident coverage plans. If you need a physical before beginning, or returning, to diving, you will need to see a physician experienced and trained in dive medicine to evaluate your symptoms or discuss health issues and diving. DAN maintains a worldwide network of doctors who serve the diving public. Upon request, DAN will provide you with contact information for physicians trained in dive medicine. If you are looking for a doctor to conduct a diving physical or would like a consultation with a dive medicine specialist in your area, call DAN at +1 (919) 684-2948 (Monday–Friday, 8:30 a.m.–5 p.m. ET).
