Visiting Nurse and Palliative Care Programs

Coping with a life-threatening or chronic illness can be overwhelming, and many people don't know where to turn for help. Fortunately for this community, Visiting Nurse and Palliative Care services are available to RCMS patients.
 
Visiting Nurse services provides care to patients who have a skilled need and are either homebound or are transitioning from the hospital, and require medication management, dressing changes, lab draws, etc. for a specific period of time.
 
Palliative Care provides a special kind of care to those who are terminally ill or near end of life. Our goal is to help our patients maintain dignity and to have the highest quality of life possible. We encourage our patients and their family to be active participants in their health care decisions.
 
These programs collaborate with patient’s primary care providers, specialists, and families to teach how to care for the patient’s acute medical needs and facilitate in-home help with other facilities.
